What is Community Care?

Community Care is an affordable health plan available in parts of Middlesex, Norfolk and Worcester counties, and in one town in Bristol County (Mansfield). The Community Care network includes Reliant Medical Group, Harrington Healthcare, Lowell General PHO, select PCPs and specialists affiliated with MetroWest HealthCare Alliance, Saint Vincent Hospital, UMass Memorial – Clinton Hospital, UMass Memorial – HealthAlliance Hospitals, UMass Memorial – Marlborough Hospital, MetroWest Medical Centers and Milford Regional Medical Center.

What do I get with Community Care?

Fallon Health’s Community Care plans include low monthly premiums, copayments and cost sharing for covered medical services and prescription drugs. Plus, you’ll get extras like:

  • The most flexible fitness benefit available in Massachusetts, reimbursing up to $150 annually for participating in healthy activities such as streaming fitness programs, Peloton subscriptions, school and town sports, gym memberships, dance and fitness classes, home fitness equipment, WW (Weight Watchers® reimagined), aerobics, Pilates, yoga classes, ski lift tickets and much more.
  • Healthy Health Plan, a program that supports members (subscriber and spouse age 18 and older) in becoming, and staying, healthy.   • Telehealth—Fallon Health’s Community Care plans cover certain services delivered via telehealth by plan providers including primary care, specialty, and outpatient mental health/substance use visits. Members also get 24/7 access to a national network of U.S. board-certified doctors from an approved telehealth vendor to discuss non-emergency conditions by phone, mobile device or online. Doctors can diagnose and treat more than 50 types of common illnesses.

Get help paying for your health insurance

Depending on your income and your household size, you may qualify to get help paying for your health insurance (subsidy). Don’t think you qualify for financial help? You might be surprised. To know if you qualify for a subsidy, please apply on the Massachusetts Health Connector website.

Looking for insurance for your small business?

If you have a small business with 1 to 50 qualifying employees and your business is located in the Community Care service area, you can purchase a Community Care plan for your employees and their families. 

  • Small businesses seeking to cover 1 to 5 employees must enroll through the Massachusetts Health Connector.
  • Small businesses with 6 to 50 employees can enroll through the Health Connector where they will receive the highest amount of tax credits, or directly through us.
